This position serves as being the community outreach coordinator between the health center referral sources to facilitate the timely admissions of residents to the health center transitional care unit. The community outreach coordinator will do this in accordance with federal, state and local standards, guidelines, regulations, and the established policies and procedures at this community. Maintains an established health center census by developing relationships with referral sources and ensuring adequate referrals are maintained.

  • Assist in the development, administering, and coordinating of department policies and procedures.
  • Implement appropriate admissions procedures in accordance with OBRA, Medicare, Medical Assistance, Blue Cross/Blue Shield, HMO/PPOs, and community policies and procedures.
  • Adhere to professional standards of conduct maintaining patient confidentiality.
  • Perform public relations, marketing and development for the community as needed.
  • Responsible for maintaining census at budgeted level.
  • Serves as public relations agent on behalf of the community with referring communities, soliciting feedback and sharing information with supervisor.
  • Represent the community at designated community groups and/or professional organizations.
  • Participates in campus marketing meetings.
  • Make presentations about the community at scheduled physician networking luncheons. Share feedback from sessions with appropriate team members and supervisor.
  • Keep supervisor apprised of prospective residents, census, and other relevant issues on need-to-know basis.
  • Work closely with health center and social service departments to facilitate efficient admission/transfer to HC from referring community to appropriate level of care.
  • Work closely with the admission coordinator regarding bed availability to facilitate admissions.
  • From time to time, provides coverage in admission department for vacation relief, etc.
  • Conduct routine onsite visits to transferring communities, meeting with appropriate personnel as required, including assessment of referred residents.
  • Complete necessary referral sheets in timely manner
  • Comply with applicable federal, state and local laws, rules, and regulations; maintains knowledge of and follows all company policies and procedures.
  • Obtain knowledge and demonstrate the principles of Living Lifespace , person directed care and successful aging philosophies and their inherent core values of commitment, respect, compassion, dedication, team work and quality.
  • Team member will support, actively participate and act in accordance with the principles of the Living Lifespace service culture and be familiar with and fluent in the language and service pillars of Living Lifespace.
  • Maintain regular and punctual attendance at work and meetings.

And here s what you need to apply:

  • Associates or Bachelor s degree required. (will vary by position)
  • Two-three years applicable experience.
  • Or equivalent combination of education and experience.
  • Has knowledge of medical terminology, treatments, PT/OT/ST functions and equipment.
  • Maintain adequate knowledge of Medicare and Medical Assistance, and various HMO/PPO level of care determination criteria.
  • Must have a minimum of two (2) years experience working in a skilled nursing community, or other related medical facility.
  • Must be able to work independent of close supervision, and accurately with frequent interruptions.
  • Must be able to make independent decisions, and deal tactfully with personnel, residents, family members, and visitors.
  • Must have good oral and written communication skills, and excellent organizational abilities.
  • Must possess a sincere desire to work with the aged and disabled, and have a good understanding of the social and psychological needs of those confined to a long term care setting.
  • Must be able to perform basic computer functions.
  • Ability to read, write and speak English sufficient to perform job duties and to interact and communicate with residents, vendors and team members.
