Schedule:
Monday-Friday
Additional requirements:
1 evening per month
2 weekend days per month (Saturday or Sunday)
Job Description Summary:
Colorectal cancer screening education, tracking, and reminder phone calls and letters.
Under general supervision, the Community Health Worker provides basic care coordination, identifies and reduces barriers to care, provides referrals to cancer screening services and necessary resources. The position will also recruit age-eligible uninsured or underinsured individuals, without regular medical checkups, for cancer screenings, HPV immunization and colorectal and lung cancer screening.
Summary:
Addresses gaps in care of patients-effectively work with individuals from diverse backgrounds in reducing cultural and socioeconomic barriers between clients and institution.
Assess and identifies actual and potential patient problems and barriers to care by gathering relative data; Identifies and assists clients in reducing barriers to care .
The purpose of this grant funded position is to conduct outreach to residents of Burlington and Camden County, enroll them in NJ CEED, and respond to barriers they face to utilizing services through the cancer screening process.
These barriers and clients are to be tracked and reports issued monthly on client progress.
Position Responsibilities:
HS diploma required
Associates Degree preferred
Prefer bilingual in Spanish or Portuguese & English
Must have a valid driver's license and access to transportation. Must be comfortable traveling within South New Jersey

- Community Outreach- provides education at events in English and Spanish, set up displays, distribute educational materials.
- Enroll clients in NJCEED , track appointments, and follow up missed appointments, mail reminder cards.
- File, create and maintain charts, general clerical duties.
- Community outreach, public speaking, knowledge of low income, uninsured communities in Burlington and Camden County.
HS diploma required
