Job Description

The Bridge for Youth (BFY) provides runaway and homeless youth with safe shelter, assists in the prevention and resolution of family conflicts, and reunifies families whenever possible. BFY strives to be the premier resource for youth and families in crisis by providing: 1) Support that empowers; 2) Safe shelter; 3) Sustainable tools for success and restoration of relationships; and 4) When possible, family reunification.

Position Overview

BFY is seeking a full-time Community Health Worker (CHW) with experience in health advocacy as a key member of BFY’s supportive housing, shelter, and outreach program teams. BFY is grounded in the belief that safe and stable housing is a key determinant of optimal health, physical development, academic success, and secure employment.

The CHW will work directly with youth in identifying health-related goals and will coordinate resources and supports with case managers, onsite mental health therapists, contracted medical providers and fellows, and more. The incumbent’s role is to provide culturally responsive education, advocate for youth and break down barriers, as well as to connect youth to health-related resources and any eligible benefits. In addition to primarily supporting youth who are pregnant or parenting, this position will also support youth who identify as LGBTQ+ and/or Black, Indigenous & People of Color (BIPOC).


Responsibilities
  • Partner directly with youth in identifying health-related goals
  • Advocate for youth and break down barriers prohibiting youth from achieving their goals. Connect youth to culturally responsive resources
  • Coordinate resources and supports with case managers, onsite mental health therapists, contracted medical providers and fellows, and more
  • Develop and lead culturally responsive health – related educational materials and support groups
  • Develop portfolio of culturally responsive health-related resources for youth who are pregnant or parenting and for youth who identify as LGBTQ+ and/or BIPOC
  • Learn relevant resources and referral streams to connect clients to eligible benefits
  • Attend regular case consults and co-create / collaborate with program teams to deliver services in a way that maximizes outcomes for youth
  • Collaborate with other community organizations and health providers
  • Maintain health-related youth records and case notes
  • Follow programmatic and organizational policies and procedures
  • Work closely with supervisor to manage tasks assigned
  • Maintain confidentiality and appropriate professional boundaries with youth and families
Qualifications
  • Experience as a community health worker, public health educator or health advocate
  • Experience working with youth experiencing homelessness, particularly those who are pregnant or parenting and youth who identify as BIPOC or LGBTQ+; Age ranges are primarily 15 – 20 years of age
  • Experience working in a housing organization or shelter with 24/7/365 supports
  • Ability to communicate fluently in Spanish and English, both verbally and in writing
  • Familiarity with client records management (CRM) databases, HMIS and/or online tools
  • Must have a valid driver’s license, access to reliable transportation with automobile insurance liability amounts to be $100,000 of bodily injury liability protection per person and $300,000 for bodily injury per accident
  • Must have ability to transport youth to appointments utilizing BFY’s vans
  • Must work a flexible schedule including evenings and weekends
  • Must clear a background check
