Community Health Care Advisor

Job SummaryUnder general supervision, provides outreach and enrollment services to Denver Health patients, families, and communities who may be uninsured and eligible for insurance programs. Assists identified families in scheduling an enrollment appointment, prescreening family prior to enrollment appointment, and filling out applications. Provides information, resources, and education regarding health insurance, accessing services, and other programs that are available.

Responsibilities

ESSENTIAL DUTIES & RESPONSIBILITIES: List each job duty and responsibility that is essential to performing the job successfully, efficiently and safely. Outreach to DH Patients, Families, and Communities • Outreach to individuals and families who are identified as uninsured through phone calls and other efforts as assigned. • Demonstrates knowledge and applicability of the procedures and requirements for enrollment in the public health programs offered. Communicates to clients expectations for financial screenings; distributes literature on programs. • Maintains updated knowledge on various insurance programs and eligibility requirements. • Returns client phone calls within one business day. • Copy materials and fax documents as appropriate. Organize and maintain files when applicable. • Participates in various community activities a various times during the week and weekends. • May outreach to and work with various community partners and agencies to schedule enrollment events including outreach involving the Health Access Express van. • Creates and maintains a secure and trusting environment by not sharing information learned on the job except when necessary in the performance of the job responsibilities or to improve a patient’s care. • CHAs that are assigned to the Health Access Express Van: drive to various community partners and community outreach activities, daily up keep, adhere to maintenance schedule, drop off van when scheduled as well as other duties assigned. Enrollment Specialist and Enrollment Event/Appointment Coordination • Collect documents from clients and complete application prescreening process, including copies of documents, placement of attest labels, and completion of application for Medicaid, CHP+, CICP, and DFAP. Place phone calls and document follow up of patients with missing documents when required. • Assists adults without dependent children with MAGI applications and delivers to enrollment. • Schedules appointments and monitors client flow based on Enrollment Specialist’s schedule and availability. Adheres to the scheduling guidelines for an enrollment specialist. • Maintain enrollment event calendars and schedules. • Communication of outcomes to various stakeholders through emails, phone calls, and reports. Collect data and update outreach outcomes in assigned database(s). Participates in team meetings and other functions as required or needed. Orient, train, and mentor new CHAs of all roles as assigned. • Promotes positive interpersonal (customer) relationships with fellow employees, physicians, patients and visitors. Treats these individuals with courtesy, dignity, empathy and respect; consistently displays courteous and respectful verbal and non-verbal communications. • Adheres to, complies with and demonstrates support for the mission and values of Denver Health. Supports and adheres to the Denver Health Dozen. • Ensures confidentiality of patient information by creating and maintaining a secure and trusting environment by not sharing information learned on the job, except when necessary in the performance of the job responsibilities or to improve a patient’s care. • Adheres to Denver Health and departmental attendance guidelines. Knowledge, Skills & AbilitiesMINIMUM QUALIFICATIONS: Education: High school diploma or GED certificate required. Experience: Typically, one year community involvement experience. Knowledge, Skills & Abilities: Knowledge of community based agencies and resources necessary to support health and health education. Interpersonal communication skills to communicate with community members, hospital staff and diverse patient population. Skill in establishing and maintaining effective working relationships with patients, employees, physicians and the public. Ability to maintain quality control standards. Ability to react calmly and effectively in emergency situations. Ability to interpret, adapt and apply guidelines and procedures. Ability to use computers and computer systems. Ability to work on holidays, weekends, and nights with scheduling flexibility. Must have reliable transportation. Bilingual (English/Spanish) preferred. Certificate/License/Registration: Colorado Driver’s License All Certifications and Licenses required for this job must be kept current as a condition of continued employment.
