Metro Nashville Public Health (MPHD) is seeking a dynamic and experienced Communicable Disease Investigator (CDI) to lead the MPHD HIV Early Intervention Services Specialist unit responsible for the linkage of newly diagnosed and lost-to-care patients to HIV specialty medical care and support services throughout Nashville and Davidson County. This position is a first-line supervisor responsible for planning, scheduling, organizing and supervising the EIS staff in our STD/HIV unit. The ideal candidate will have experience and/or knowledge of effective supervisory and training skills, experience in public health field work, and knowledge of e HIV testing, transmission, control, investigation in a community health program as well as experience linking HIV patients to medical care and support services.

- Performs lead or supervisory responsibilities for the education of EIS specialists concerning patient education, intake assessment and patient evaluation, the Ryan White RWES enrollment and linkage-to-care (LTC) processes, CAREWare data entry, HIPAA compliance, patient communication and case closure procedure.
- Oversees EIS participation in available testing, street outreach, HIV educational activities, LGBTQIA outreach programs, e.g. PRIDE and health fairs.
- Coordinates with HIV Surveillance unit to monitor and assign newly diagnosed and lost-to-care patients to EIS specialists as well as coordinates with and facilitates the Disease Investigator Specialist (DIS) unit’s location of and communication with HIV patients and the documentation of their treatment.
- Monitors all MPHD clinic data to ensure that both newly diagnosed, and lost-to-care HIV patients are linked to medical care within 30 days of their latest specimen date.
- Responsible for reviewing case analysis and documentation along with ensuring daily clinic, field and surveillance activities occur according to local and state protocols.
- Oversees patient data entry into CAREWare and ensures that data is accurate, complete, routinely entered, and up-to-date, and coordinates with Ryan White Part A epidemiologist to generate required reports, reduce errors and streamline data entry process.
- Plans and manages daily staff assignments and assists staff with difficult, problematic and/or complex cases.
- Is assigned and maintains a patient caseload that is commensurate in size with other supervisory duties.
- Plans and carries out training and development of employees through unit meetings and training opportunities, such as HIV conferences and symposiums, online webinars and local HIV educational events.
- Identifies and test persons at high risk of acquiring communicable disease.
- Constructs and maintains EIS forms and How-To Manual and updates as needed.
- Perform other related duties as assigned.
Graduation from an accredited college or university and two (2) years of successful progressively responsible full-time paid employment in related field required. One (1) year within the past two (2) years in a public administration/supervisory capacity preferred. Completed graduate degree in a directly related field may be substituted for one (1) year of the required experience. Possess required competencies upon hiring.
Valid class “D” driver’s license, use of personal vehicle, and maintenance of valid personal vehicle insurance as required by Tennessee Law.
