Responsibility:

  • Provide personal care and related services for hospice patients and assist the family with patient care.
  • Provides direct and personalized care based on the patient’s individualized Aide Care Plan, determined by a Registered Nurse. If aide care plan is not clear, or patient’s condition has changed, aide is responsible for reporting immediately to a Registered Nurse for reassessment.
  • Assists the patient to attain and maintain good personal hygiene.
  • Encourages the patient to be as independent as possible and maintain Assisted Daily Living (ADL's), according to the patient's abilities and condition.
  • Offers emotional support to the patient/family, as needed.
  • Communicates always to the patient/ family with respect and dignity. Documents all interactions and collaborations timely and accurately on every visit/ phone call performed.
  • Communicates to Registered Nurse or clinical staff to provide and replenish any medical supplies and equipment, as directed.
  • Reports any change in the patient's mental, emotional, and/or physical condition to the designated clinical interdisciplinary team members (nurse, social worker, chaplain, manager), as directed.
  • Completes documentation accurately, concisely and in a timely manner, as appropriate.
  • Documents patient visit actions and interventions performed and not performed with reasoning not completed in the electronic patient record.
  • Contributes to overall team functioning by responsible participation.
  • Actively participates in weekly Interdisciplinary Group (IDG) meetings, when possible.
  • Actively participates in Organizational and Team meetings.
  • Actively attends additional continuing education, in-service, and/or community event opportunities, when possible.
  • Performs other duties as needed and as directed by clinical management.
  • Completes successfully and timely at least 12 hours of continuing education per calendar year.
  • Completes timely and professionally all needs required by the clinical manager or Registered Nurse to ensure Quality of Care, Infection Control, Compliance, and Customer Service Improvement initiatives are met.
  • Completes all mandatorily assigned tasks and modules before the designated deadline.
  • Completes successfully all the packet forms and material requirements outlined in the Annual
  • Competency Evaluation.
  • Communicates directly with supervisor/ manager any variations in scheduling, visit requests, customer nfeedback, or other needs immediately after change awareness.
  • Other duties as assigned

Supervisory Responsibilities:
This job has no supervisory responsibilities.
Education/Experience:
High school diploma, or general education degree (GED); one to three months related experience and/or training or
equivalent combination of education and experience; successful completion of a nurse aide training program that meets
State and Federal requirements; and, good standing on the Florida State Nurse Aide Registry.
Language Skills:
Communication skills of reading, writing, and speaking in English. Bilingual (fluent in Spanish) a plus.
Math Skills:
Math skills including addition, subtraction, multiplication, division, and percentages/fractions.
Certificates, Licenses, Registrations:

  • Certified Nursing Assistant (CNA) meeting the requirements of State of Florida and Medicare regulations. .
  • Valid Florida Driver's License.
  • 75 hours of Home Health Aide (HHA) certification.
  • BLS CPR Card
  • Certification as a Hospice And Palliative Care Nursing Assistant (CHPNA) is required after 500 working hours or 1 year of employment (criteria met first). Job role allows, after criteria met and before two years, certification is obtained.

distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. Driving to patient
homes during day and night hours is required.
