JOB DESCRIPTION: Direct Care Staff (1099 independent contractor)
PROGRAM: HolBrock Estates Supportive Housing
EXEMPT/NON-EXEMPT: Exempt
SUPERVISED BY: Operations Manager (Assisted Living Manager)
COMPENSATION: $10.10-$11.00 To be compensated on the bi-weekly check cut schedule, you must submit your completed service invoice to the supervisor by the end of the service day.
WORK SCHEDULE: The CNA/ CMT works various shifts. May be scheduled to work shifts that are rotating, may be asked to work longer shifts. The following shifts are available. 7am-4pm Saturday and Sunday
3pm-12am Saturday and Sunday
7am-7pm Saturday and Sunday
7pm-7am Saturday and Sunday
ANTICIPATED TRAVEL: 5% of the time. There is no additional compensation for travel.
SUMMARY OF POSITION RESPONSIBILITIES: The role of the CNA is to provide the programs clients with supportive services, supervision, personalized assistance, health-related services, or a combination of these services to meet the needs of individuals who are unable to perform, or who need assistance in performing, the activities of daily living or instrumental activities of daily living, in a way that promotes optimum dignity and independence for the individuals.
COMMON MISCONCEPTIONS WHICH MAY REQUIRE CLARIFICATION:
- So long a resident is in the facility in which the direct care staff is assigned, the direct care staff must be present.
- Regardless of the time of day services are rendered, the direct care staff is to remain awake.
- The direct care staff person may not leave the assigned site during the service time block until all services are rendered at specified times herein.
- You may only provide services one time 12-hour time block during a 24-hour period unless approved by the supervisor.
- The company does not provide contractors supplies, tools or transportation to perform the work. Supplies are issued to the clients, and contractors are expected to use the client’s supplies to service that client. Clients generally keep supplies issued to them by the company in the common areas.

REQUIRED CERTIFICATION/LICENSE:
- CNA/GNA/CMT, must have the appropriate Maryland Board of Nursing certification.
- 80 assisted living manager certifications preferred but not required
- Drivers license preferred but not required
PHYSICAL DEMANDS: Regularly walk, stand or stoop; occasionally lift, carry, push, pull or otherwise move objects weighing up to 25 pounds; and regularly drives a motor vehicle.
WORKING CONDITIONS: Work is performed in an office, in client’s homes or in the community; Work exposes incumbent to contagious or infectious diseases; Work may expose incumbent to dangerous and volatile situations; which could result in bodily injury.
LEVEL OF EDUCATION/TRAINING/QUALIFICATIONS:
- Reliable transportation to the service site
- High school diploma or GED
- Must be 18 years old or older
- Must be free from tuberculosis, measles, mumps, rubella, varicella and any impairment which would hinder the performance of assigned responsibilities
- Have no criminal convictions or criminal history that indicates behavior that is potentially harmful to residents, as evidenced through a criminal background check completed within 30 days before employment;
- Have sufficient skills, education, training, and experience to serve the residents in a manner that is consistent with the philosophy of assisted living;
- Participate in an orientation program and ongoing training to ensure that the residents receive services that are consistent with their needs and generally accepted standards of care for the specific conditions of those residents to whom staff will provide services;
- Receive initial and annual training from the Company in:
(a) Fire and life safety, including the use of fire extinguishers;
(b) Infection control, including standard precautions, contact precautions, and hand hygiene;
(c) Basic food safety;
(d) Emergency disaster plans; and
(e) Basic first aid by a certified first aid instructor;
(f) The health and psychosocial needs of the population being served as appropriate to their job responsibilities;
(g) The resident assessment process;
(h) The use of service plans; and
(i) Resident's rights; and
(j) Shall demonstrate competence in personal care services and infection control protocols to the delegating nurse before performing these services;
(k) Basic CPR and First Aid;
(l) Cognitive impairment and mental illness
SERVICES (to be performed on all shifts):

1 Check in with shift supervisor at the start of the shift
2 Collaborate with staff leaving duty, complete staff collaboration form and submit to supervisor
3 Ensure all clients receive morning medications and document whether or not clients took medication on the MAR in icaremanager. Secure the medication cabinet and return the key to the supervisor. Notify supervisor if medication errors or medication is running low.
4 Review with each client the clients treatment, services and activity schedule for the day. Update the clients calender and information board calendar as needed. Issue 3-ply appointment slips for new appointments. Schedule / coordinate transportation for clients appointments today with logistics coordinator / supervisor / UberHealth / MTA mobility / client's preferred method of travel.
5 Write care notes on each client residing in the facility in ICan Notes. Update the resident status on clients if clients are on LOA, hospitalized or no longer reside on site.
6 Verify in I Can Notes under resident agreement section that each client has a resident agreement on file. If a resident agreement is not on file, send an email to housing and notify the supervisor.
7 Verify in I Can Notes under RAT section that each client has a RAT done by the delegating nurse within the past 45 days prior to today's date. If there is no RAT or the RAT is outdated, send an email to the DN and notify the supervisor.
8 Verify in ICan Notes under service plan section that each client has a service plan done by the delegating nurse within the past 6 months days prior to today's date. If there is no service plan or the service plan is outdated, send an email to the DN and notify the supervisor.
9 Be available for the emergent needs of the clients answering questions and troubleshooting as needed
10 Ensure all clients receive breakfast, lunch, and dinner in the dining area and document clients’ food intake in icaremanager software. Notify supervisor if there is not adequate supply of food for 72 hours. Wash dishes and clean dining area.
*if no dietary staff present, prepare the meal or assist the client with meal preparation
*if dietary staff are present, serve the meal once prepared by dietary staff
11 Ensure all clients receive afternoon medications and document whether or not clients took medication on the MAR in icaremanager. Secure the medication cabinet and return the key to the supervisor. Notify supervisor if medication errors or medication is running low.

13 Verify in I Can Notes under fire drill section that your facility fire drills are up to date. That means on all shifts, a fire drill has been completed for each quarter. Shift A is defined as the time period between 7:00 AM to 3:00 PM. Shift B is defined as the time period between 3:00 PM to 11:00 PM. Shift C is defined as the time period between 11:00 PM to 7:00 AM. Quarter 1 is defined as January to March. Quarter 2 is defined April to June. Quarter 3 is defined as July to September. Quarter 4 is defined as October to December. If the fire drills are not compliant send an email to housing detailing the SHIFT and QUARTER missing then notify the supervisor.
14 Verify in icaremanager under disaster drill section that your facility disaster drills are up to date. That means on all shifts, a disaster drill has been completed for each semi-annual period. Shift A is defined as the time period between 7:00 AM to 3:00 PM. Shift B is defined as the time period between 3:00 PM to 11:00 PM. Shift C is defined as the time period between 11:00 PM to 7:00 AM. Semi-annual period 1 is defined as January to June. Semi-annual period 2 is defined as July to December. If the disaster drills are not compliant send an email to housing detailing the SHIFT and SEMI-ANNUAL PERIOD missing then notify the supervisor.
15 Ensure bathroom, bedrooms, kitchen and common areas clean and sanitized - at minimum: spray mattress, linens and pillows with lysol and bedbug spray, clean touchables with bleach/lysol, sweep/mop floors, spray chairs/sofas with lysol and bedbug spray
16 Be available for the emergent needs of the clients answering questions and troubleshooting as needed
17 Perform any work related task requested by the supervisor and enforce policies and procedures. Notify the supervisor if any visitor comes to the site prior to allowing the visitor to enter. (Activities, servicing the clients spiritual needs, and/or if applicable food/supply ordering may be done at this time at the request of the supervisor)
18 Ensure all clients receive snacks in the dining area and document clients’ food intake in ICan Notes software. Notify supervisor if there is not adequate supply of food for 72 hours. Wash dishes and clean dining area.

*if dietary staff are present, serve the meal once prepared by dietary staff
19 Walk-thru facility and ensure: ___menu is current and posted, ___ licenses are current and posted,___ all equipment/furniture is in tact, ___there are no pests or hazards, ___ activity schedule is current and posted, ___refrigerator/freezer thermometer are reading correct temperature ranges, ____each resident has all the furnishings and items they require. For any missing items in the above list, send an email to housing and notify the supervisor.
20 Collaborate with staff entering duty, complete staff collaboration form and submit to supervisor
21 Check out with supervisor at the end of each shift
