General Overview

We are seeking an experienced Clinical Social Worker for our Family Medicine Office located at 130 County Road
in Ipswich. The practice is open Monday through Friday 8:00am to 5:00pm.

Family medicine practices treat patients of all ages. They offer annual check-ups and age-appropriate screenings and vaccinations, as well as preventive medicine. Family medicine practices enable an environment where the providers have a more detailed understanding of a family's health history and the health dynamics between generations, which helps them provide more consistent care.

In alignment with the North Shore Medical Center Mental Health team, as well as the integrated Care Management Program (iCMP), our NSPG Clinical Social Workers are woven into our Primary Care model and leveraged as key members of the care team. Working collaboratively with other members of the patient’s care team, our Clinical Social Workers are involved in assessment and triage of patients and families, to ensure provision of appropriate, timely, and effective evaluation and treatment.

Working within our practices provides our Clinical Social Workers with the freedom of an autonomous work-setting as the in-house Mental Health subject matter expert paired with the resources/support of a larger health system to empower them in their clinical decision making.

• Minimum of 2 years clinical experience, preferably post-Masters experience. Other experience may include post acute care settings or care coordination/case management experience.
• Minimum of Masters-level degree program in social work
• Current licensure in MA in social work

Preferred

• LICSW, will provide supervisory hours required for candidates to obtain this certification
