The Clinical Practice Performance Analyst (CPPA) will support clinical relationships and engagement with clinical transformation consultants (CTCs) and physician practices participating in UnitedHealthcare's (UHC) Accountable Care program, Value Based Programs, or Population Health Management programs such as Patient Centered Care Model. The CPPA will also partner with internal United Healthcare shared services (such as Medical Directors, Health Plan Market Leaders, Provider Advocates, and Networking) with a goal of improving health and practice performance while reducing medical costs. As the single point of clinical contact for a practice, the CPPA, will be accountable for the full range of clinical practice performance which may include but is not limited to improvement on HEDIS and STARs gap closure, coding accuracy, facilitating effective education and reporting, and proactively identifying performance improvement opportunities through the use of data analytics, technology, workflow changes and clinical support. These roles support the development of comprehensive, provider-specific plans to increase their physician practice performance, reduce readmissions and improve their outcomes.

Primary Responsibilities:
- Build and effectively maintain relationships with practice leadership and key clinical influencers actively involved in practice transformation
- Coordinate, schedule and prepare joint operating committee (JOC) meetings between executive practice leadership, practice staff and UHC Accountable Care teams
- Provide practice facing, onsite support to improve quality and utilization metrics
- Support clinical transformation consultants (CTCs) within assigned region in closing gaps in care and other duties as assigned
- Monitor and review the progress of the practice in milestone achievement and insure the practice is accountable for successful completion
- Use data to analyze key cost, utilization and quality data and interpret results to assess the performance of the practice
- Train clinic and office staff on UHC tools and processes
- Monitor and report progress on assigned tasks of internal teams and practice commitments
- Consult and partner with internal UHC matrix partners and the practice to identify organizational and structural challenges hindering achievement of desired program outcomes
- Collaborate with UnitedHealthcare teams including the practice Care Coordinators, quality management teams, hospital clinical teams, behavioral health teams to support integrated PCP driven care for our members with practices and hospitals

- 2+ years of experience with data analysis and reporting
- Ability to build a positive working relationship with transformation consultants and practices you are assigned
- Ability to build a positive and collaborative relationship with Commercial, Medicaid and Medicare local and National Functions (Contracting, Product, and Operations)
- Ability to support market UHC health plan and accountable care community through events extending resources to address care needs for Commercial, Medicare and Medicaid
- Potential to progress and obtain industry certifications
- Access to reliable transportation and valid driver’s license
- Willing to Air Travel expected less than 25%

Preferred Qualifications:
- 2+ years of combined experience with health care analytics, or driving clinical transformation initiatives with population health programs, Patient Center Medical Homes or in the managed care industry
