Provides supervision to Hospice team of registered nurses, medical social worker, aides, bereavement coordinator, office staff. Staff provides services to Hospice patients in the home, skilled nursing facilities and inpatient. Submits required documents to payers, oversight of timely filing of HIS, acts as a resource to all staff related to Hospice and Palliative Care. Participates in operations, quality and other committees, projects and other tasks to support Agency needs. Manages payroll, human resources for Hospice team and all other clinical coordinator responsibilities. Makes on site supervisory visits with staff to observe competencies. Stays current regarding Hospice regulations, industry trends and issues. Actively participates in DPH, JCAHO,Baldrige, Magnet surveys,and audits, Responds to patient and families needing additional information about services and acts as a model of professionalism and customer service to her staff.
Educational / Minimum Requirements:
Graduate of an accredited LPN program. Two years in Occupational Health Clinic setting.
State/Federal Mandated Licensure or Certification Requirements:
LPN licensure
Bristol Hospital Mandated Educational Requirements:
General orientation at time of hire. Fire/Safety/Infection Control annually. Other programs as mandated by Hospital.
Special Requirements:
Experience in Hospice/Palliative Care, strong assessment skills, interpersonal skills, management experience
Physical Requirements:
Exposure to extended walking and standing.
Work Environment:
Exposure to patient elements; exposure to corporate client work sites.
Cognitive Requirements:
Basic math skills, ability to accurately document results of patient encounters.
