Working toward the goals of ending HIV as an epidemic in the United States and improving the long term health outcomes of people living with HIV, Housing Works provides a comprehensive range medical, behavioral health, vocational and support services to people living with HIV/AIDS as well as innovative prevention support for individuals at high risk for contracting HIV. The Be InTo Health project, a partnership between Housing Works and NYC DOHMH, is a specialized intervention aimed at helping HIV positive Black and Latina cisgender, transgender, non-binary and/or genderqueer women reengage in HIV care and other critical support services. Working as part of the interdisciplinary team that includes a Project Manager, Clinical Social and an RN, the Client Navigator will work with assist with the recruitment, enrollment and delivery of enhanced navigation services, including helping clients access support services, entitlements and benefits, transportation services, accompaniment to appointments and other service interventions that promote retention in care and activate clients in the management of their own healthcare. Ongoing training on the Be InTo Health intervention will be provided by Housing Works and DOHMH, however, the ideal candidate will have experience working with high risk populations, including but not limited to HIV, homeless, substance use and mental health. Additionally, candidates should have a high degree of cultural competency in working with cisgender, transgender, non-binary and or genderqueer Black and Latino women.


Responsibilities:

  • With support of Clinical Care Manager, make referrals to connect clients to needed services, including health care, food and nutrition, transportation and other services.
  • Verify engagement in HIV treatment and if needed provide timely reconnection to care.
  • Collaborate with interdisciplinary team on the development and delivery of individual and group structured health education Work with Data Assistant to identify clients who may be eligible for Be Into Health Services and initiate contact with potential participants.
  • In collaboration with Project Manager and Clinical Care Manager, develop outreach and engagement strategies that effectively communicate benefits of program enrollment.
  • Based on goals identified on the plan of care, assist clients with navigation of the health care and social service systems.
  • Provide and follow though on referral of clients to any needed services.
  • Conference with client’s internal and external providers to coordinate services and assess progress toward care plan goals.
  • Confirm with health care provider that STI screenings are provided semi-annually or as needed.
  • Assist to maintain an environment that is sensitive and welcoming to a diverse client population, including but not limited BIPOC, transgender and non-binary individuals.
  • Participate in departmental Quality Assurance and Quality Improvement (QA/QI) activities.
  • As part of the integrated care team, participate in interdisciplinary team meetings and care coordination activities.
  • Participate in required trainings or meetings with program administrators and contract funders.
  • Other responsibilities as needed or required.
Minimum Requirements
  • High School degree or equivalent with demonstrated experience providing health education, or certification as a health educator, and/or peer with relevant lived experience. Or Bachelor’s Degree in human service field with relevant experience.
  • Cultural competency with Black and Latina, cisgender, transgender, non-binary or genderqueer women.
  • Motivational interviewing training helpful but not required.
  • Ability to work in a team management and interdisciplinary system.
  • Understanding of HIV/AIDS and its impact on homelessness, substance use and mental illness.
  • Bilingual (English/Spanish, English/Haitian-Creole) preferred but not required.
