Job Description
Merakey is currently seeking Clinical Assessors (Fee for Service) to join our Behavioral Health Division in our Brentwood, TN location.
Hours Available in Davidson County.
Schedule: Fee for Service/ Varying Schedules
Compensation: $28.00 per hour
Location: 5110 Maryland Way Brentwood, TN 37027
POSITION OVERVIEW
The Clinical Assessor will travel to various sites to conduct assessments with new and existing clients. For follow up hospitalization assessments, the employee will be responsible for ensuring that the consumer has an appropriate and attainable discharge plan in place.
Responsibilities include but are not limited to the following.
Merakey offers a continuum of care in behavioral health (includes mental health and addictive diseases), integrating services in both community and site-based settings. These services provide a wide range of recovery focused treatment, services, and supports for children and adults.

Responsibilities include but are not limited to the following.
- The Clinical Assessor must be (LPC or LCSW) and will complete Assessments, Follow up Hospitalization Assessments and Behavioral Health Assessments within the community. This position is primarily community-based and requires daily travel.
- Keep open communication with Coordinators and /or Administrative staff on referrals for potential and existing adult clients.
- Compare individual assessments by traveling throughout the community to meet with clients, their families, and/or others involved in their treatment and services.
- Determine, based on the outcome of the completed assessment, whether existing clients meet the criteria for continued placement or services and for new clients determine whether placement or services is medically necessary.
- Make recommendations as to what provisions the facilities should have in place to meet the clients' needs.
- Work a flexible schedule at times as per business necessity.
- Work independently and with minimal supervision.
- Maintain complete, timely and appropriate documentation per program requirements. Complete reports and correspondence as needed.
- Provide consultation to staff with regard to client care and treatment.
- Adhere to all Merakey, County, State, and Federal policies and regulations related to mental health and addiction treatment services.
- Complies with Merakey ethics and code of conduct, as well as the ethics and code of conduct of own discipline, in contact with all clients, co-workers, and the community.
- Participates in regular clinical supervision and continuing education activities.
