Overview:
We are looking for a Clinical Abstractor to join our growing Community Health Partners team! Community Health Partners - part of Community Health System - helps primary care physicians and specialists manage their practices. You will be responsible for analyzing, presenting, collecting and submitting high-quality clinical data to program-specific registries. As part of the team associated with clinical data, you will work closely with various clinical departments, including physicians helping identify opportunities for quality improvement based on needs.

Responsibilities:
In your role, you will:
Identify patients for inclusion in the program registry through the application of strict program inclusion/exclusion criteria and randomization protocol.
Ensure reliable collection of data components for the program through the effective utilization of the hospital medical record system.
Utilize knowledge in metric/measures definitions to speak to identified outliers. Attend conference calls and provide education to appropriate leadership.
Demonstrate applicability of the methodology and the reliability of definitions utilized by reviewers within the program through Inter-Rater Reliability (IRR) testing.
Identify areas for streamlining and process improvement in the data collection process. Demonstrate appropriate utilization of resources necessary to obtain valid, reliable data for entry into the Program. Qualifications:

Education

Successful completion of a nursing education program required
Bachelor's Degree in Nursing preferred
Master's Degree in Nursing preferred

Experience

3 years experience in inpatient nursing or hospital based nursing required
Data analysis and clinical chart review experience required
Database data entry and/or management experience. Statistical knowledge. Quality improvement or patient safety knowledge and experience preferred

Licenses and Certifications

RN - Current State of California Registered Nurse license required
