Job Description
Community Care is hiring a Clinic RN to join our team. This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, being part of a mission driven organization and who shares our mission to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This is a part-time position 32 hours a week Monday – Friday No holidays or weekends
Under the supervision of the assigned Clinic Manager and under the direction of primary care, is responsible for assessing, coordinating, monitoring, and providing clinical treatment and services required by Community Care program members.
Clinic RN Responsibilities
Required - experience including 1 year of experience providing care or services for a frail or elderly population. Excellent clinical and assessment skills. Knowledge of multiple systems of care or adults and persons with physical disabilities.
Preferred - Previous clinic or urgent care experience preferred.
Clinic RN Care Education
Bachelors of Nursing preferred
Licenses/Certifications
Additional Requirements
Telephone Triage Certification within three (3) years of hire preferred.
Clinic RN Job Schedule:
Monday to Friday 7:30AM – 2:30PM
