Under supervision: Responsible to perform initial assessment and development of individualized service plan for PAS clients. Responsible for establishing frequency of client supervisory visits based on the individualized need of the client and to assure appropriate performance of the attendant. Under no circumstances may a Supervisory visit be performed less than once annually per client no more than 90 days before the end of the annual period and no less than 30 days before the end of the annual period. Responsible for performing supervisory visits as frequently as necessary* to o assure service delivery §* address EVV discrepancies *§ verify attendance and proper clocking in of attendants for service delivery o address the needs of the client o assure adequate training and performance of the attendant Responsible for recruitment, orientation, and training of PAS Attendants. o Responsible for assuring an attendant is hired to meet the requirements of assigned clients o Responsible for training attendants regarding rules, regulations and policies, o Responsible for training attendants how to use the EVV system to log in and out o Responsible for teaching safety, procedure, tools, and preferences for each assigned task o Responsible for telephone orientation of qualified special backup attendants as appropriate Responsible for coverage of PAS hours and coverage of service interruptions. o Monitors EVV information and works with EVV staff to respond to incidents where time is not logged, or autolink fails to occur o Inspects and identifies issues with EVV devices and utilization of devices o Completes appropriate forms and follows up to assure replacement of faulty devices and to assure attendants us appropriate compliance processes until the EVV device is replaced o Recruits, hires, places and orients attendants Caprock Home Health Services, Inc Position Description 10/2016 o Provides services directly when lack of an attendant will result in failure to provide personal care services necessary to protect the health and safety of the client. Maintains communication with all members of the client care team, including all members of the PAS staff and skilled clinicians when appropriate. Responsible to contact clients routinely via telephone or personal visit to assess and document: o Client service plan is adequate. o Client continues to need services. o Client needs service plan change. o Attendant continues to be competent in providing authorized tasks. o Attendant is delivering authorized tasks. Responsible for observation, reporting and documentation of client’s status. Responsible to maintain current, accurate and thorough documentation to support services, compliance, billing and payroll functions, as well as information for agency reports. Responsible to monitor and direct operation and effectiveness of infection control and OSHA standards. Maintain knowledge of PAS licensure and program regulations. Cooperates with all staff members in carrying out the overall objective of the agency. Performs other duties assigned by Supervisor. Job Skills and Abilities: Must possess the ability to perform and demonstrate personal care tasks with proper technique, body mechanics, and infection control measures. Must be able to assess the competency of attendants in the performance of personal care tasks. Must possess strong organizational skills. Strong written and oral communication skills required. Requires the ability to learn, comprehend and retain licensure and program specific rules and regulations as they pertain to PAS services. Ability to use and learn computer software programs including Vesta Microsoft Word, Outlook, and Excel. Must learn and use current client management system software. Requires a valid Texas driver’s license. Qualifications: General: Good verbal and written communications skills in English. Good driving record. Clear Criminal Background Record. Ability to communicate in Spanish a plus. Physical: Must be able to physically perform personal care tasks in and about the homes of clients. Must be able to communicate verbally, use a keyboard, sit for long periods of time. Must be able to get in and out of a car, and to drive a car to and from clients Caprock Home Health Services, Inc Position Description 10/2016 homes, enter various homes, navigate stairs and various dwellings. Must be able to turn and position clients weighing up to 200 pounds alone, and heavier with the use of a hoyer lift or assistant. Must be able to lift and carry up to 30 pounds. Education, Training, and Professional Credentials: A licensed nurse or an individual with a high school diploma or GED and two years of full-time study at an accredited college or university. An individual with a high school diploma or general equivalence diploma (GED) may substitute one year of full-time employment in a supervisory capacity in a health care facility, agency, or community-based agency for each required year of college. Prefer experience in the home health field. Experience: Prefer two years of supervisory experience.
