Homes for the Homeless, Inc. (HFH) is seeking Case Managers to assist families with children residing in our shelters in setting goals, tracking progress, and connecting to services. This position requires strong organizational skills, attention to detail, sensitivity for issues surrounding poverty, and good written and verbal skills. Maintains and updates case files to document progress and facilitates information sharing with other staff members. Openings are available in the North and South, Bronx and Jamaica, Queens.


ABOUT HOMES FOR THE HOMELESS, INC.

Homes for the Homeless, Inc. (HFH) is a non-profit social services organization operating family shelters throughout NYC that serve as a hub of opportunity for families both residing in shelter as well as those in the neighboring community. On-site programs range from childcare, early childhood education centers, afterschool clubs and programs, and youth recreation activities, such as sports teams, to housing assistance, case management, parenting workshops, career and job vocational training, support groups and substance abuse assistance, and family engagement and recreation events for the whole family.


ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Maintains and updates digital and physical records of all families as required by governing agencies
  • Collaborate with clients to develop service plans based on an assessment of each family’s needs
  • Assisting families with referrals to outside agencies for employment, education, parenting, legal, medical, and mental health services
  • Maintains and updates case files bi-weekly to document progress
  • Assisting families in moving to permanent housing
  • Ensuring that clients maintain an open public assistance case
  • Adhere to all city and state guidelines for maintaining documents
  • Conducts unit inspections

QUALIFICATIONS

  • Bachelor’s degree
  • A desire to work with families in need
  • Familiarity with case management, entitlement systems, and a knowledge of issues that impact families is a plus
  • Knowledge of Outlook, Word, and Excel
  • Bilingual in Spanish or other language is a plus
