SCHEDULE: PRN/Per Diem
The RN Case Manager provides hospital case management/utilization review and discharge planning to assure that the patient progresses through the continuum of care, and is discharged to the least restrictive environment. Medical Center of Trinity is a 340 bed, all-private room hospital located in the rapidly growing tri-county area of Pasco, Pinellas and Hillsborough counties. Planned housing communities, schools, ample shopping and nearby beaches and recreational facilities make it an excellent location for young families. The hospital opened in February 2012 in a beautifully designed and spacious facility, with services including adult and pediatric emergency care, spine & orthopedics, women’s health & obstetrics, heart & vascular including open heart surgery, robotic assisted and minimally invasive surgery, men’s health, cancer care, behavioral health, surgical specialties, primary care and neurosurgery. Medical Center of Trinity is a certified Primary Stroke Center and supports three freestanding ERs. The hospital also features an Accredited Comprehensive Community Cancer Program (CoC) andhad the top performing HCA Nursing Unit of Distinction in 2018 and the #2 Unit in 2019.

Qualifications

Licensure/Education:Current State of Florida RN licensureCompletion of an RN Diploma program or related healthcare programNational Case Management Certification strongly desire Qualifications:Minimum of three (3) years recent experience in an acute-care hospital setting requiredCase Management experience is preferred
