Case Manager (Registered Nurse)

SCHEDULE: Full-time, Days (with rotating weekends)

$7,500 sign-on bonus available for eligible new hires!

POSITION SUMMARY: The RN Case Management Care Coordinator provides ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. Enhances the quality of patient management and satisfaction to promote continuity of care and cost effectiveness through the integration of case management, utilization review management and discharge planning. QUALIFICATIONS:
  • Registered Nurse (RN) diploma program or completion of related health care program required
  • Minimum of three years recent acute clinical practice or related health care experience required
  • Florida RN license required; or appropriate compact license; or ARNP
SKILLS, KNOWLEDGE & ABILITIES: Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills, Demonstrates analytical and critical thinking abilities with pro-active decision-making and negotiation skills, strong knowledge of governmental and private organization and community resources.
