Case Manager - Low Barrier and SES Focus
Provide a broad spectrum of services including non-clinical case management, resource management, and community support. Provide specialized intervention services to people who inject or are at high risk to inject drugs.
Responsibilities:
- Case management, including developing treatment plans, case coordination, screenings & intake, and documentation
- Establish relationships and partnerships with local resources in order to increase access to harm reduction, MAT services and other grant related deliverables
- Crisis Prevention and management
- Clinical documentation and reporting
- Acquiring, stocking and distributing Narcan with training
- Identify and provide outreach to individuals with limited resources who are in need of treatment
- One year experience in related field
- A valid driver's license and use of a personal vehicle
- The availability of working evenings and/or weekends
- Full time (37.5 hours)
- Exempt
