Position Summary: The Case Manager (CM) is responsible for providing case management support, which include home-based visitations and office consultations ensuring that the basic needs of residents are met. The CM is also required to work in collaboration with other Dignity Staff to provide residents with comprehensive services and supports. The position is full-time.
Shift: 35 hours per week, Monday - Friday, with hours in the office and offsite.
Qualifications:
Bachelor’s Degree required; with 2-year experience preferred. Familiar with standard case management procedures, documentation and reporting; strong knowledge of government benefits and community resources within a social service environment; must have strong background or experience with substance addiction; strong knowledge of domestic violence and mental health pathologies; proficiency in most general computer operation systems and applications required, including: Word, Excel, Outlook, and Client Database. Excellent time management and organizational skills; excellent written and oral communication skills; highly energetic and creative; able to work in a team; passionate about improving the lives of others while maintaining appropriate boundaries.
Essential Duties and Responsibilities:
Conduct a basic resident background analysis and needs assessment for Dignity residents using information from residents’ files and consultation with other staff. A resident background analysis and needs assessment must be conducted within 30 days after each resident has been admitted into the program.
Maintain contact with residents (weekly, bi-weekly, or monthly and complete progress notes as required. Complete and submit monthly statistical/data reports and quarterly service delivery narratives that capture current data on residents and make special note of barriers, progress, and interventions within assigned caseload. The CM is required to conduct at least one (1) in-home, at work, and/or in-office consultation with each client every month.
Maintain uniform, up-to-date resident records as required by the agency and funding sources.
When appropriate, assist other staff members to develop and implement workshops or other presentations that enhance the residents’ ability to achieve objectives in their action plans. Encourage residents to participate in varied workshops or special events sponsored by Dignity Housing or other appropriate entities.
Participate in evening meetings and/or tenant events, as scheduled.
Participate in monthly supervision meetings with the Executive Director to review, assess and ensure appropriate supportive services delivery.
Participate in monthly Social Service Division meetings to review overall program and organizational progress and coordinate service delivery efforts.
Wherever feasible, appropriate, and authorized by the Executive Director, identify other funding, technical and community resources that support the ability of the agency to respond to tenant needs.
Complete and submit other documents as required by the Executive Director.
Other such duties as needed and instructed by supervisor.
On the first day of employment, Performance Goals for the first 6 months will be provided.
