Job Description

Houses of Hope’s TASC program is hiring for Intensive Care Managers.
In this fast paced, dynamic position you will work with consumers whose lives have been impacted by multiple, serious, chronic behavioral health issues. TASC's case workers are empowered to work across organizational and disciplinary boundaries to help keep consumers supported and stable in the community.

Job Summary:

Intensive Care Management is a planning and problem-solving function that overcomes obstacles faced by high utilizers identified from within the behavioral health system, with goals of improved consumer outcomes and reduced overall costs to the behavioral health system. In contrast to traditional case management approaches, which are often tied to single clinical disciplines or to a single service system, Intensive Care Managers are intended and empowered to cut across organizational and disciplinary boundaries.

Minimum Qualifications:

**Per Executive Order 14042 all of House of Hope employees are required to be fully vaccinated for COVID-19. Proof of vaccination is required.

Master’s degree or Bachelor’s degree with an equivalent of three to five years experience in human service related field. Prefer three to five years experience in case management. Possess skills needed to work with broad range of individuals (gender, race/ethnicity, substance abuse and mental health patients, family members, other professionals). Must be able to work independently and as part of a team, have good organizational skills and be available to participate in an on-call rotation. Knowledge of human service/social service network preferable.
