Job Description
To provide on-going support and expertise through comprehensive assessment, planning, implementation and overall evaluation of an individual's needs. The overall goal is to promote coordination of care, accountability of care and discharge planning. Targeted case management assists beneficiaries to design and implement strategies for obtaining services and supports that are recovery oriented and individualized.
Essential Functions:
Provides case management and discharge plan development to assure individual progresses through the CMH continuum of care and is transitioned to the least restrictive treatment environment.- Responsible for the coordination of mental health, physical health as well as social service supports as a foundation for the case management service.
- Immediate response (within 24 hours of admission to the mental health unit or within the next business day for meeting) to begin active participation in the discharge planning process with the inpatient treatment team and treatment meetings. These may require travel and generally occur in Jackson and Hillsdale counties, but travel within the State of Michigan may be necessary.
- Immediate response to individuals in caseload who are in crisis or have urgent needs. Ability by phone and if needed respond face to face. These may require travel and generally occur in Jackson and Hillsdale counties, but travel within the State of Michigan may be necessary.
- Provide support and linkage to appointments post discharge. These may require travel and generally occur in Jackson and Hillsdale counties, but travel within the State of Michigan may be necessary.
- Ensure completion of Medicaid application as well as any other entitlements the individual may be eligible for; this should happen immediately upon provision of case management services.
- Provides screening, assessment/risk management, care planning implementation of plan of service, monitoring and evaluation of progress and advocacy for the individual.
- Assures the plan of service occurs through a person centered planning process.
- Promote engagement of consumers with crisis services and after care treatment through utilizing company vehicle and following company transportation procedures when needed and necessary.
- Travel is required to manage situations, generally occurs in Jackson and Hillsdale counties, but travel within the State of Michigan may be necessary at unexpected times to meet the needs of community members and consumers.
- Work a non-traditional schedule which may include evenings, weekends and/or holidays.
- Adhere to productivity standards as they relate to your caseload.
- Complete weekly supplemental documentation including contact log and productivity, as well as bi-monthly supplemental documentation including consumer not seen list.
- Responsible for adherence to all Commission on Accreditation of Rehabilitation Facilities (CARF) and Michigan Department of Health and Human Services (MDHHS) standards.
Education/Certification/Licensure:
- Bachelor's degree in human service field, required.
- Credentialing requirements for ongoing clinical privileges with LifeWays.
- CPI non-violent physical crisis intervention training, upon hire.
- Mental Health First Aid Training, upon hire.
- Must have reliable transportation*
- LifeWays vehicles are provided to employees for the purpose of transporting consumers; employees must possess a valid MI motor vehicle operator's license and verification of an acceptable driving record. Personal vehicles may be used for company business when drivers meet the following; valid MI motor vehicle operator's license and proof of personal vehicle insurance in accordance to guideline.
