Job Description
Position Summary:
Responsible for coordinating the clinical management of patients and families with specific health care needs through collaboration with other members of the healthcare team. Plans, organizes and directs home care services and is experience in nursing with emphasis on community health education and experience.
Job Requirements:
Minimum Education:
- Graduate of Accredited School of Nursing
Minimum Work Experience:
- 3 Years preferred with 1 year in a home health setting
Required Licenses/Certifications:
- Registered Nurse
- CPR - BCLS
Location: Hendrick Health · H/C Housecalls Med Skilled Nsg
Schedule: Full Time, Day Shift, M-F 8a-5p with on call rotation
