Job Description

Case Manager - Housing Specialist

Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time Housing Specialist to join our talented multi-disciplinary team in our Community Support Program. The schedule for this position is Monday-Friday, 8AM-5PM.

The Housing Specialist Case Manager will be responsible for implementing and supporting the Permanent Supportive Housing program. The Case Manager will work collaboratively with staff, agencies and services providers to develop and provide stable, affordable, and long-term independent housing opportunities through short and long-term subsidies such as the Housing Choice Voucher program or other types of permanent housing. The Case Manager will provide assistance to adults with serious and persistent mental illnesses who may experience homelessness.

Responsibilities

  • Schedule, coordinate and facilitate housing opportunities for individuals through housing development activities which may include outreach to landlords and other individuals to market the housing program, conducting housing presentations, and help to complete applications for various types of housing resources.
  • Assist individuals secure and maintain affordable housing utilizing search activities such as contacting prospective landlords to schedule showings, scheduling housing interviews for individuals, assisting with rental application process, and helping individuals make informed decisions.
  • Provide housing assistance to ensure successful execution of a subsidy contract and to maintain current information about individual’s benefits to ensure that subsidy amounts are accurate with regard to the individual’s income.
  • Provide housing advocacy and other support services to individuals
  • Prepare reports which may include demographic information, services provided to individuals, and appointments with individuals, landlords, property managers, housing subsidy administrators, and marketing efforts.

Requirements

  • Bachelor’s degree in Psychology, Social Work, Education, Rehabilitation, or Human Services required.
  • One year’s human services experience preferred.
  • A valid driver’s license, personal vehicle, and auto insurance is required.
  • Excellent communication, problem-solving, time management, and organizational skills are required.
  • Availability to work some flexible hours to provide on-call support and respond to after-hours concerns and emergencies is required.

All staff are required to wear face masks when in common areas of our facilities or when in close proximity to any other individual. To promote social distancing, staff may work staggered shifts and/or work in the office on alternating days, or work remotely in certain circumstances. Riverbend provides personal protective equipment (PPE) to staff appropriate for their type of service delivery. Clinical services are being provided via telehealth, telephone, or in-person depending upon the needs of the client and the job. Not all jobs can offer telehealth and/or remote working options. We are taking all necessary precautions for any in-person service delivery. Riverbend takes great care to ensure that frequently touched surfaces are being sanitized throughout the day, ventilation systems are functioning at optimal levels for maximum air circulation and quality per CDC guidelines, and each location is fully sanitized daily.
