Position Summary

The WellSpace Health Case Manager will be a member of the discharge planning team whose primary function is to assist with appropriate discharge and treatment planning of WellSpace Health patients who are hospitalized or seen in the Emergency Departments to create consistent, connected care in both in-patient and out-patient settings. This includes providing access to on-going medical and behavioral health care, as well as, providing connections to important community resources.

Required Qualifications

  • High School Diploma or equivalent required.
  • AA degree in human services field, Medical Assistant diploma, or AOD education OR a combination of work or lived experience sufficient to successfully perform the essential duties of the job.
  • Consistent, reliable attendance and punctuality during peak hospital usage hours as scheduled is a mandatory requirement for this position.
  • Basic knowledge of substance abuse, mental health, and medical issues and treatment; Knowledge of community resources.
  • Ability to establish and maintain positive, professional relationships with all hospital, community agencies, and WellSpace health stakeholders.
  • Able to function effectively in a fast paced environment.
  • Ability to write progress notes and maintain detailed, statistical records.
  • Report monthly on statistics of the program.
  • Demonstrated commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired.
  • Must be able to maintain appropriate boundaries with clients.
  • Communicate clearly and concisely, both verbally and in writing and demonstrate a high level of listening skills.
  • Ability to abide by standards of professional ethics and maintain HIPAA rules of confidentiality.
  • Computer (PC) literate in Microsoft Word, Excel, Powerpoint, Outlook email and database software.
  • Must have a reliable vehicle, and be willing to transport clients as needed or determined by supervisor or program Manager.
  • Must have valid driver’s license and insurable driving record.
  • Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
  • Assist and support the Center/Department/Program to meet standards of High Reliability.

Essential Responsibilities

  • Participate with the hospital team planning for an appropriate discharge and treatment care plan for each identified patient. This will include attending the complex care meetings when requested.
  • Engage each identified and referred patient within 24 hours on weekdays to participate in a needs assessment with the team in order to identify appropriate services for all co-morbid conditions. As directed by the team these may include current level of functioning, patient input, cultural needs, housing, identification of natural supports and any other needs expressed by the patient or the team;
  • Create a treatment plan with the patient and hospital team to assist with successful follow-up and care prior to and once discharged.
  • Once the patient has discharged, engage patients out in the community within 24 to 48 hours to provide intensive outreach services including connections with any follow-up medical care appointments or medical related needs.
  • Provide transportation assistance to ensure patient accesses community resources and medical appointments;
  • Based on the deliverables the Case Manager will report all outcomes as determined by BHI Grant data reporting requirements.
  • Attend calls and/or meetings with staff to communicate current patient needs and foster on-going consistent communication between WellSpace Health and area hospitals.
  • Attend bi-monthly performance, administrative supervision with Case Management Supervisor.
  • Other duties as assigned.

Physical Demands and Work Environment

The work environment is characteristic of a medical clinic environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands to handle, or feel. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.


The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bodily fluids (blood, urine) and hazardous chemicals.
