Overview:
As a Case Management Navigator with our Case Management team the average day is anything but average and you will actively feel and see how your role impacts our mission and those we serve. You will join a dedicated team of professionals with a fervor for doing good work in our community by screening patient census on a daily basis for appropriate discharge planning interventions, assessing patient discharge planning needs, and escalating patients to appropriate Case Management discipline based on Discharge Navigator – Escalation Criteria .

In this role you will work collaboratively with the Case Management staff and other health care team members to coordinate and execute discharge planning services for patients through the healthcare continuum. You will help assist in the execution of a plan of care to facilitate a safe discharge in a timely manner and serves as a liaison on community facilities and resources. With a patient ratio of 1:45 your day is anything but average.

Experience and education minimum requirements:
Bachelor's Degree in Social Work or Healthcare related field or Completion of an accredited Vocational Nursing School program
2 years of experience in Case Management with prior assessment and referral experience required
BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
LVN - Current State of California Licensed Vocational Nurse License for incumbents without a qualifying Bachelor's Degree required
Preferences: • Experience in discharge planning in an acute care hospital or skilled nursing facility preferred
