Purpose

The Care Manager (CM) is an important team member supporting the Nursing department, which is essential for achieving the dual mission of FMRI. The CM shall be accountable for 1) aid FMRI’s patient population navigate the transition from the inpatient service to treatment at an FMHC outpatient facility or another source of care. 2) The CM shall be responsible for tasks in the arenas of both Transitions of Care and Care Management.

The CM works closely with physicians and midlevel providers, scheduling staff, medical records staff, Community Health Workers, as well as team members from the Pharmacy and Business Office departments.

In addition to our great benefits, FMRI Clinics operate Monday – Friday during business hours with select clinics open for a weekly evening clinic until 8:00 pm and Saturday morning rotations.

Education:

  • A.D.N. or B.S.N. from accredited College of Nursing.

Experience:

  • Minimum of two years clinical practice; previous population health, care management or transitions of care experience preferred.

Licensure / Certification:

  • Valid license to practice as a Registered Nurse (RN) in the state of Idaho, current BCLS certification, and Certified Case Manager certification preferred.

Credentialing:

  • Meets established Credentials Committee requirement for Registered Nurse scope of practice including fully credentialed, licensed and privileged as well as the ability to pass a background check.

Other Knowledge, Skills and Abilities:

  • Ability to interact professionally and courteously with varied members of the medical community as well as members of the general public.
  • Must have strong customer service and interpersonal skills, possess the ability to collaborate with other health care providers in the provision of patient care, be able to delegate to other team members as required, and have the ability to multitask in a busy environment.
  • The ability to function in stressful and emergency situations is absolutely essential, as is compliance with the American Nurses' Association Code of Ethics for Professional Nurses.
