Position: Care Manager

Reports to: Lead Supervisor

Position Type: Full Time, Exempt

Office Location: Racine, WI

Location of Caseload Coverage: Racine & Kenosha County


SUMMARY:

As a member of an Interdisciplinary Team, you will provide on-going care management of adults with disabilities and frail elders through regular face-to-face and phone contact, following the standards set forth by the Managed Care Organization. You will develop and maintain care plans that assist Members to live and function as independently as possible. Collaborating with the Family Care RN on the Interdisciplinary Team to assure the health and safety of Members enrolled in the Family Care program.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Complete a comprehensive psychosocial assessment, identify and mitigate potential health and safety concerns, and develop a member-centered plan of care collaboration with the Member and Registered Nurse focused on achieving and sustaining Member outcomes and fostering independence.
  • Encourage Members to take an active role in decision-making regarding their long-term care plan designed to assist them to live and function as independently as possible at home and in the larger community.
  • Through the development and evaluation of the member-centered plan, identify Member outcomes and the most effective and cost-effective interventions, including paid and unpaid services, needed to support those outcomes.
  • Provide assistance to Members and their families to access resources identified to support outcomes.
  • Coordinate and authorize services in the benefits package that are necessary and appropriate to support Member outcomes in the most cost-effective manner as well as oversight of supports provided through natural supports.
  • Oversee the effectiveness of the established member-centered plan and interventions through regular phone and in-person contact with the Member and their support system. Re-evaluate and adjust the plan during major life changes to assure to plan and interventions are tailored to the unique needs of the Member.
  • Respond to and report any event, incident, action, or in-action that jeopardizes the health and safety that involves a Member to the MCO and establishes a plan to prevent future reoccurrences.
  • In crisis situations, provide an appropriate assessment of psychiatric, behavioral or other concerns, and implement interventions in conjunction with the Registered Nurse, Supervisor, additional MCO staff, Member supports, the Member’s physician and/or other community support professional.
  • Maintain accurate documentation of care management activities in Member records promptly, thoroughly and professionally in accordance with the standards set forth by the MCO.
  • Participate as needed in MCFC’s quality assurance and utilization review programs and follow quality standards of practice established by the MCO.
  • Attend team meetings, staffings, staff development programs, and in-service training and periodically collaborate with professionals who have additional specialized expertise when needed to assist in assessment, consultation, ongoing coordination efforts, and other areas as needed.

EDUCATION and/or EXPERIENCE:

  • Bachelor’s degree from an approved college or university required with a major in a social services area (e.g. social work, rehabilitation, psychology) or a Wisconsin social worker certification.
  • At least one (1) year of paid formal (non-family setting) experience working with specific groups, including adults with IDD, PD and Frail Elder, in social service care management or related social service experience with persons in the MCO’s target population.
  • For a bachelor’s degree in any other area, the candidate must have a minimum of three (3) years of paid formal (non-family setting) experience with people in the MCO’s target population.
  • Valid Driver’s License with access to a reliable vehicle
