Position Summary

The purpose of this role is to facilitate appropriate care delivery to patients with the goal of keeping the patient safely at home. In order to provide appropriate care to meet the needs of the patient, Constellation is utilizing Telemonitoring and advance practitioner telemedicine/video visiting as well as other Best Practice approaches to avoid unnecessary hospitalizations. This role is designed to support these processes including installation and setup of telemonitoring, tracking inventory etc. This individual will also provide administrative support to the Care Manager to ensure that the essential functions are executed timely. This may include coordinating NP virtual visits, assisting with technology to facilitate video visiting to the patient by any clinician, tracking/review of records, updating of logs, etc.

Essential Functions

  • Install/set up Telemonitoring Devices as assigned
  • Assist patients with taking readings as assigned
  • Track inventory/support ordering process as assigned
  • Coordinate virtual visits
  • Travel between our 3 office locations, Norwalk, Orange & Wethersfield.
  • Provide technology support to patients/families for telemonitoring, virtual visits
  • May be assigned to perform other duties as directed by Manager
  • Although each position has its own unique duties and responsibilities, the following applies to all employees of Constellation Health Services.

Qualifications

  • Bachelor’s Degree preferred but will consider strong candidates with HS Diploma/GED
  • 2-5 years of relevant experience and/or training
  • Experience in a healthcare setting. Strong preference will be given to those with Home Health and/or Hospice experience.
  • Excellent computer, organizational and time management skills required.
  • Ability to work independently on assigned tasks as well as to accept direction on given assignments.
  • Professional appearance, proven track record in providing excellent customer service.

Physical Requirements

1. Hearing and speaking ability is required to communicate on the telephone.

2. Vision is necessary for entering data.

3. Reaching is required for paper management.

4. Manual dexterity is required for operating the computer, photocopier, and telephone.

5. Mobility is required for going to photocopier, fax machine, etc.

6. Must be able to drive to and from the office to patient homes and inter office travel.

7. For training and competencies must be able to operate a hoyer lift

8. Must be able to speak and write the English language in an understandable manner
