Care Coordinator

Bachelor’s Degree required, Social Work or related field preferred.

P*OSITION SUMMARY: * The Care Coordinator is responsible for offering emotional and practical support to seniors they do this through information and referral, providing socialization calls, conducting support groups and helping develop peer support groups. The incumbent consults with seniors in the community to determine their needs, coordinate services, educate them on the necessary steps required to obtain services and become more active in East River senior services. The Care Coordinator will display a compassionate nature, be knowledgeable about the senior population, and provide exceptional customer service to seniors.

• Addresses non-case management related requests and information and referrals.
• Consults with seniors to discuss their needs and education them on potential East River services for which they can engage to improve their quality of life. Collaborate with other internal and external resources to set up appointments and meetings to assist the senior as needed.
• Identify barriers that may be preventing seniors from engaging in East River services and develop and consult with the Director of Senior Connections to create an action plan to make East River services more accessible and appealing to seniors.
• Check-in on seniors regularly and evaluate and document their progress with obtaining services or being actively engaged in socialization activities.
• Plans and utilizes resources effectively to achieve optimal coordination of care. Service and responds to questions, issues and/or concerns the seniors may have.

Job Type: Full-time

Pay: $45,000.00 - $53,000.00 per year
