We are hiring for a:
- Position Title: Blended Case Manager - FRANKLIN and/or FULTON COUNTY
- Department: Targeted Case Management - Mental Health
- Status: Full Time/Non-Exempt
- Days/Hours: 37.5-hour workweek
- Starting Pay: $17.00/hrly, extra .48/hr for master's degree in related field

Summary of Responsibilities:
Under the general direction of the Blended Case Management Supervisor, this person provides Blended Case Management for persons served. He/she is proficient in the use of a personal computer, and is adept at handling multiple tasks at one time. This person has expertise in providing good customer service.
This person is expected to exercise initiative and judgment in discharging his/her responsibilities. Duties are conducted in accordance with all existing policies, procedures and regulations and in accord with the Mission and Values of the Company.
Essential Functions:
Builds relationships, Leads teams to support the people we serve, Collaborates with individuals, families and providers to ensure people are living their best lives, Facilitates meetings and discussions, Advocates, Critically thinks and problem solves, Provides crisis intervention, Links individuals to community resources, Assesses needs, Completes and updates Individualized Service Plans, Completes casenotes, Be on-call.

Education and Experience:
Bachelor’s Degree in psychology, social work, counseling, social welfare, sociology, vocational rehabilitation or related field. MH/DD experience, preferred.
Employment Requirements:
Automobile and Valid Insurance, Valid Pennsylvania Driver’s License and Acceptable Motor Vehicle Record Check, Act 33 and Act 34 Clearances, FBI Fingerprint Clearance, Social Security Card, Immigration and Naturalization (I-9), Drug Screening Clearance.
Working Conditions:
Prolonged periods working on a computer in an open office environment, comfortable communicating through virtual technology, and travel throughout the assigned region may occur and may be in adverse weather conditions, must be able to lift up to 15 pounds.
