Department Int Med Infectious Diseases 50000589 Business Unit College of Medicine Reg-Temp Full-Time Regular Work Schedule TBD by supervisor and responsibilities Position SummaryThe Bilingual Medical Case Manager will provide various levels of case management services to individuals living with HIV/AIDS including assessment, referral and support for participants of Ryan White grants. This position will assist patients enrolled in Ryan White services with medication adherence through pharmacy support. This position will also assist with housing instability, food insecurity, mental illness and other issues through a referral network. This position will assist patients with prescription submission, Ryan White renewals, and complete enrollment into eligible prescription, State, Federal and community programs.
