Your day.
As a Bariatric Navigator, you need to know how to:
- Serves as a patient liaison to help patients and family members navigate the complex healthcare system as it relates to weight loss surgery.
- Function as a resource to patients, physicians, and the bariatric program for assigned bariatric site(s) through all phases of bariatric and weight loss surgery by coordinating a system-wide delivery of service expectations.
- Establish early contact with bariatric patients to identify and eliminate barriers to timely diagnosis and treatment.
- Assess emotional, spiritual, psychosocial, financial and other patient needs.
- Provide coordinated patient care by actively collaborating with other members of the healthcare team and case managing patients.
- Provide direct patient and family education, as well as community outreach, education, and screening in conjunction with the RN’s in the team.
- Serve as a resource with expertise in Bariatric care management in education with patients and families.
Your experience.

- High School Diploma or equivalent, required
- Bachelor's degree in healthcare or related field experience, preferred
- Minimum of three (3) years’ experience in ambulatory or post-acute care setting, required
- Care management experience, preferred
