Description:

The RestorixHealth At-Home Program is a unique model combining direct patient care in the patient’s home by physicians and nurse practitioners with live telehealth support. Using a centralized call center, mobile electronic health record, wound care specialty support and evidence-based treatment protocols, the at-home providers consistently deliver the highest-level outcome-based wound care. This means faster initiation of care as patients don’t wait for appointments in physician offices or hospital outpatient departments, reduced instances of patients missing appointments as the care provider comes directly to the patient, more control over the care process and reduced patient self-referrals to hospital emergency care.


Job Summary:

This position will help support all aspects of care within the organizations and providers scope of practice. The NP will provide for medical care to patients as outlined in their collaborative agreement under the oversight of their supervising physician in the outpatient, inpatient, home or nursing home setting. Knowledge and advanced skills with advanced physical assessment and treatment of complex wounds, management of skin integrity and chronic care management is required


Primary Responsibilities:

  • Evaluates and sets a treatment plan for all initial and established wound referrals at the patients place of residence.
  • Covers all patient locations within assigned territory
  • Accurately performs history and physical examinations on patients with acute and chronic wounds.
  • Assess, diagnose and manage chronic conditions for patient population.
  • Identifies deviations from normal, initiates and evaluates treatment for patients with commonly occurring health problems within scope of practice.
  • Consults, co-manages or refers as appropriate.
  • Prescribes medications as delegated in the protocols and/or procedures.
  • Provides individualized patient teaching and counseling.
  • Communicates with PCP, referral physician, and/or office staff regarding any changes in patient’s condition.
  • Communicates with collaborating provider as outline in collaborate agreement and state guidelines.
  • Completes necessary documentation as outlined in RXH Policy.
  • Develops, implements and/or participates in research or educational activities.
  • Maintains competency in clinical practice and meets and maintains requirements for all certifications.
  • Ensures proper documentation of diagnoses and procedures performed to allow for

appropriate coding.

  • Assess and creates treatment plans and prescribes medication in accordance with state requirements and organization protocols.
  • Initiates evaluation and treatment procedures essential to appropriate response to emergency wound and ostomy situations.
  • Participate in education of patients as well as Managed care organizations, nursing home and RXH staff to assure best outcomes and most current clinical practice. Ensures full availability during hours assigned/agreed upon with RestorixHealth At-Home leadership.
  • Orders and manages supply inventory required for At-Home services
  • Performs other related duties as assigned.


Additional Responsibilities:

  • Participates in quality improvement activities and monitoring of health outcomes for patients
  • Actively participate in creating and implementing improvements to achieve clinical, satisfaction and/or efficiency outcomes.
  • Work to ensure the continuity of the mission, vision, and values of the hospital.
  • Responsible for ensuring current state license


Education and Technical Skills:

  • Masters or Doctoral Degree as a Nurse Practitioner
  • Valid state nursing license
  • 2 years’ experience working in an acute care setting as an APRN or RN in one of the following departments: Wound Care, Emergency Room, and or Medical/Surgical Unit
  • Excellent interpersonal skills
  • Board Certification in any of the following Adult Nurse Practitioner/ANP, Family Nurse Practitioner/FNP, Geriatric Nurse Practitioner/GNP, or Clinical Nurse Specialist/ CNS
  • Prescriptive authority
  • BLS from American Heart Association

  • Excellent interpersonal skills
  • Strong organizational and time management skills preferred


Physical Requirements:

This position requires the physical demand of standing, bending, lifting, stooping, pushing, pulling, and kneeling. Some work may require moderate physical exertion (30-50 pounds) on a continuous basis (over 50% of time) or heavy physical exertion (over 50 pounds) on a frequent basis (up to 50% of time).
