Summary:

An individual trained in basic spiritual care assessments and interventions. Completes basic spiritual care assessments and care delivery to patients, families and associates. Assists individuals with the integration of spiritual and cultural traditions into the plan of care. Offers basic support and counseling during crisis and non-crisis events. Is inclusive of individuals of all faiths and no faith; works to facilitate communication between individuals and their faith communities. Refers complex cases and ethical issues to a staff or clinical chaplain.Responsibilities:
  • Assess, provide and documents effective spiritual care to patients, families and associates including listening, counseling and prayer.

  • Supports individuals from diverse economic, cultural, racial and religious backgrounds; advocates for patient needs, values, and goals in the care process.

  • Provides calming support for individuals in crisis, undergoing procedures, facing end-of-life decisions and traumatic events.

  • Proactively participates in interdisciplinary delivery of holistic patient care services through rounding, departmental meetings and family conferences to promote optimal and realistic goals and outcomes.

  • Responds to patients, families and associates with ethical decision-making, values and discernment makes.

  • Participates and leads spiritual care groups.

  • Assists with the implementation of advanced directives.

  • Seeks consultation from staff or clinical chaplains if uncertain on how to respond to patient needs.

  • Leads worship and administers sacraments in accordance with personal faith tradition and denominational approval as appropriate.

  • Coordinates spiritual connections with pastors and spiritual care advisors in the community as requested.

  • Participates in department meetings, organizational sponsored programs and educational secessions as requested.

  • Consistently follows policies, procedures and approved practices.

  • Incorporates all patient safety protocols and initiatives to optimize a safe patient environment.

  • Maintains patient confidentiality of personal, medical information.

  • Uses information technology and various electronic systems to assess, document and communicate patient care and work.
  • Performs other duties and responsibilities as assigned.
Qualifications: Education: Bachelors Degree required
  • Bachelors degree required; Theological or Healthcare care education preferred.
  • In lieu of degree, equivalencies for the undergraduate level theological education would be acceptable for faith traditions that do not offer degrees. Declaration and documentation of equivalencies are required.
  • Must be involved with a recognized faith group.
  • Training in pastoral ministry and/or hospital chaplaincy. Ecclesiastical endorsement required.
  • One unit of Clinical Pastoral Education (CPE) or equivalent preferred.
  • Must attend CISM training at least every two years.
