Position Summary:

Maintain company-wide wellness program. Creates awareness, motivation and provides tools to employees that help them adapt and maintain a well-rounded, healthy lifestyle. Provide education, coaching curriculum, and coaching to help participants identify their motivations, wellness visions, and build care plans that will help them achieve their long-term goals.


Essential Duties and Responsibilities


  • Assessment of medical reports for patient and calculation of scores for employees; evaluate need for continued follow up with employee based on scores from physician report.
  • Actively engage with members via a variety of communication tools including telephone, email, text, and 1:1 coaching session to provide feedback and support, answer questions, share resources, and drive patient engagement to promote healthy daily habits and set attainable goals through diet, medication management, exercise
  • Educate and counsel patients emphasizing personal motivation to improve behaviors, education and awareness to improve the lifestyle behaviors as well as set goals, answer questions, and provide guidance and on-going support
  • Maintain, coordinate and update wellness programs information and procedures to meet client specifics.
  • Utilize the established template and documentation standards
  • Prioritize and coordinate multiple responsibilities
  • Work closely with the clients to update on benefits of program and identify trends
  • Maintain confidentiality and compliance with Federal, State and local regulations pertaining to HIPAA
  • Other duties as assigned by supervisor
  • Some travel may occasionally be required to meet with clients

Education:

  • Certified Health Coach from an accredited organization (NCHEC or NBHWC) or Nurse Case Manager, Exercise Physiologist, or Nurse / Medical Case Manager.
  • College degree preferred, with minimum 2 years of health coaching experience, or related experience.


Experience:

  • Two or more years of recent Wellness Coaching experience.
  • Experience working with chronic condition management, such as diabetes, hypertension, weight management, etc.
  • Bilingual preferred

  • Excellent customer service skills to calmly and professionally resolve participant concerns with coaching and/or technology
  • Strong verbal and written communication skills
  • Maintain confidentiality- Knowledge of laws and regulations pertaining to HIPPA and PHI
  • Must be open to feedback and have a desire to continuously learn and grow
  • Proficient with Microsoft Office applications including Word, Excel, and Power Point
