Position Summary: Under the direct supervision of the Community Response Manager and the Director of Community Response, the Violence Prevention Navigator (VPN) will support survivors of domestic violence (DV) / inter-personal violence (IPV) referred by medical providers, providing them emergency response, emotional support, strengths-based case management, resources and referrals. The VPN will also be responsible for outreaching to medical providers, health care systems, Federally Qualified Health Centers (FQHC), and other community partners as needed to raise awareness about DV, IPV, and WEAVE’s services.
Essential Functions:
Direct Services:
- Provide immediate response to medical facilities requesting support for their patients.
- Provide emotional support, follow-up services, and Strengths Model case management to survivors who have experienced DV / IPV referred by medical providers, or as assigned.
- Attend medical community partner DV meetings (UCD, Sutter, Kaiser, etc.).
- Build partnerships and referral systems for medical providers screening children and youth for Adverse Childhood Experiences (ACEs) who have experienced or witnessed domestic violence.
- Actively develop and maintain working relationships with local health organizations and agencies according to VPN Strategic Plan.
- Guide medical partners to WEAVE services through outreach, presentations, referral system, etc.
- Assign referrals received through the Unite Us portal.
- Assist clients with establishing a medical home and obtaining health insurance.
- Serve as the primary Advocate responding Advocate for Domestic Assault Forensic Exams (DAFE) during weekday business hours.
- Provide back up to the primary Sexual Assault Response Team (SART) Advocate during weekday business hours.
- Facilitate peer support groups at various community partner locations, as needed.
- Provide individual peer support when requested.
- Contribute narrative to quarterly progress reporting.
- Communicate with WEAVE’s Community Response Manager for support and supervision, as needed.
Other Responsibilities:

- Attend various internal and external meetings as required
- Complete all necessary data entry, forms, case records, and statistical reports accurately and timely.
- Submit required documentation to the supervisor/manager within the designated timelines.
- Fulfill contract obligations and requirements.
- Assist in the development of presentation/training curriculum when needed.
- Attend and actively contribute to supervisory and team/unit meetings, internal training sessions, conferences, seminars, and independent study.
- Adhere to professional standards and initiate requests for assistance from Supervisor to address new issues or complex concepts.

Qualifications:
- Combination of education and experience to successfully fulfill position requirements, lived experience valued
- Experience with crisis intervention around domestic and sexual violence and sex trafficking preferred
- Ability to establish and maintain effective working relationships with representatives from community partners and social service agencies
- Self-directed and self-motivated required
- Experience with Microsoft Office (Word, PowerPoint, Excel) and data entry
- Excellent oral and written communication skills
- Regular attendance
- Consistent demonstration of positive, cooperative, self-motivated, and professional behavior
- Work with individuals with different areas of expertise, backgrounds, knowledge, and skillsets
- Ability to be a team player and commit to working together to reach goals
- Complete WEAVE’s 69-hour training and certification program (post-hiring requirement)
- Provide a copy of valid California driver license, have reliable transportation, and submit current proof of vehicle insurance
- Satisfactory results from DOJ background check and TB Test (post-employment offer requirement)
ADA Job Characteristics:
Work environment is an office setting with the delivery of services at emergency shelters expected. Local travel (driving) will be required to attend off-site meetings, response services, and client transport. Must be able to lift and carry 25 lbs., extend both arms above the head and/or reach below the waist, stoop, squat, crawl, bend the back to open lower-level filing drawers, or retrieve items, climb a ladder or stairs, or use an elevator. Must be able to sit frequently and/or stand when filing, using a copier, etc., and use fine manipulation (hands and wrist) to operate a keyboard, take notes, utilize desk calculator, file, and copy documents. Need corrected vision to normal range frequently for reading the monitor and hard copy documents. Speak in normal vocal tone and range and communicate clearly and concisely to others. Apply above-average reasoning and detail as required in the accomplishment of job duties.
