Position Responsibilities:
Service Activities:
- Conduct assessment interviews with potential Veteran clients
- Assist Veterans with the creation and implementation of Individualized Service Plans (ISP) which incorporate housing, employment/income and financial goals, recovery for both SUD and Mental Health and individualized crisis management.
- Track Veterans' progress and modify ISPs at minimum monthly, as grant guidelines require.
- Assist Veterans in accessing ISP goal-based services in the region.
- Assist with coordination of resources within the continuum of care.
- Provide in outreach to the Veteran population in need of case management services.
- Facilitate housing search with Veterans clients. (when applicable)
- Provide transportation for veterans to and from the facility, appointments, housing searches, etc.
Clinical Activities:
- Conduct initial assessments and develop goal plans in collaboration with the Veteran and other provider organizations including the Veterans Administration.
- Conduct psycho-social assessments and develop treatment plans in collaboration with the Veteran and other provider organizations including the Veterans Administration.
- Follow the models of Motivational Interviewing and Harm Reduction.
- Coordinate referrals of Veterans to the Veterans Administration and other community-based providers for additional services based on needs and eligibility (this includes expanding the existing referral network as necessary).
- Maintain case records in accordance with program guidelines and professional requirements.
- Implement legal and ethical standards of service delivery, privacy, and confidentiality, complying with all applicable regulations.
- Assist with team treatment planning for veterans across the organization
Program Development:
- Perform case finding functions and outreach, identifying and screening veterans for case management needs.
- Coordinate supportive services with applicable providers including the Veterans administration.
- Ensure that Veterans Place services are utilized as part of the provider network along with other Community providers.
- Oversee the delivery of services within the scope of this program and make recommendations to The Veterans Resource Center Manager.
- Work closely with the VA HCHV walk-in and crisis center, Allegheny Link and other homeless outreach services to ensure a high quality of services.
- Development and implementation of a group dealing with barriers to housing weekly
Communication:
- Prepare reports as requested by other Veterans Place and Veterans Administration staff.
- Participate in administrative, staff, clinical, and in-service activities and training as required.
- Prepare written reports and complete documents as requested.
- Communicate policies and the rationale behind them to program participants.
- Inform other agency staff members of relevant matters related to the program.

Qualifications:
- Bachelor's Degree in related field required or equivalent experience, Master's Degree preferred
- Must be proficient in Microsoft Office Suite including Word, Excel, and PowerPoint
- Experience working directly with the homeless population strongly preferred
- Experience working with Veterans Administration preferred
- Must have a valid driver's license
- Veteran Status preferred
- Passing a pre-employment drug and alcohol screening is required and a background check will be conducted
