- Obtains patient name and verifies patient room number/location and transfer destination. Checks patient’s name band before transport.
- Ensures that all checklists and permits are signed before transporting patient.
- Recognizes special isolation precautions and procedures when transporting patients, seeks clinical guidance as needed.
- Prepares patient for transfer and transfers patient to stretcher/wheelchair using proper body mechanics when ambulating, moving and/or lifting patients.
- Observes patient during transport and reports any change in patient condition or questions to receiving nurse/tech.
- Provides safe patient transport and demonstrates appropriate use of side rails up, wheel chair foot rests, legs, and brakes. Positions patient for comfort as condition allows and obtains supplies for patient comfort (warm blanket, urinal, etc.). Maintains patient’s right to privacy during transport.
- Ensures oxygen tanks are are placed in holders and that tanks are returned to designated area(s). Assess need for valve replacement on oxygen tanks. Notifies Respiratory Therapy with assistance with trachs. or any patient on a mask ventilation.
- Tags out malfunctioning equipment and removes from use for repair.
- When returing patients from transport ensures bed is in position, plugged in and I.V. pumps are plugged in.
- Ensures catheters and drains are positioned to facilitate drainage during and after transport. Ensures Sequential Compression Devices are removed and re-applied as necessary.
- Ensures Central Monitoring is aware that a patient is being transported.
- Cleans wheelchairs and stretcher and applies clean linen after use.
