Position Summary:

The transition specialist provides our students with opportunities to pursue their interests and develop vocational skills through on-site and community work experiences. The transition specialist provides on-the-job training and assessments at various worksites both on- and off-campus to students attending the pre-WAGES and WAGES programs. The transition specialist assists with the development and implementation of the consumers’ supported employment plans and vocational outcomes. Also assists in vocational assessment of students aged 14 and up.

Principle Duties and Responsibilities:

  • Maintains written program updates and progress notes and reports progress by phone, e-mail or in person.
  • Provides information about the WAGES programs to local businesses and employers.
  • Provides community-based assessments to students enrolled in the WAGES programs.
  • Provides individual supervision and support for students in their place(s) of employment.
  • Assists special education teachers in the development and implementation of students’ individual vocational IEP goals and objectives.
  • Develops individualized IEP goals based on transition to adulthood and employment in concert with the IEP Coordinator.
  • Supports academic activities essential to employment within the WAGES classrooms including reading- and math-skills, communication and social skills, following directions, completing job applications, etc.
  • Attends semi-annual reviews and annual IEP meetings for individuals enrolled in the WAGES programs. May attend other meetings, as requested.
  • Acts as a liaison between student employers, students, teachers and support staff and families/house mangers/care providers.
  • Provides job coaching services and supports for a designated caseload. This can include: on-site and vocational-skills assessments, support to improve work behavior and work-related social skills, vocational skills training, mobility training, work area adaptation, co-worker support, training and student advocacy.
  • Informs student educational team members of concerns with student job-site placements or employer issues.
  • Provides support to students so they may develop vocational skills and understand the responsibilities and benefits that come with working in their community.
  • Any other duties as designated by the Assistant Director of Education.
  • Other duties may be assigned by the Assistant Director of Education.

Qualifications, Skills and Knowledge Requirements:


  • Bachelor’s Degree in Special Education, Nursing, or a Human Services field with one (1) year experience as a teacher of the disabled or as a provider of treatment or case management services for the disabled and their families; or
  • Associate’s Degree in Special Education, Nursing, or a Human Services field with three (3) years’ experience as a teacher of the disabled or as a provider of treatment or case management services for the disabled and their families; or
  • An equivalent combination of training and experience.
  • Knowledge of OPWDD regulations and NYSED policies and procedures preferred.
  • Duties require professional verbal and written communication skills.
  • Proficiency in or knowledge of using a variety of computer software and e-mail applications, especially Microsoft Excel, Outlook and Word; have the aptitude to learn other computer software as necessary.
  • Must be able to lift 50 lbs. independently and assist in lifting weights exceeding 50 lbs.
  • Meet the requirements to drive Springbrook vehicles as required.
  • Be able to perform all physical interventions.
