.Week day hours available between 8:00 am - 4 00 pm No weekends Independent Senior Living communities The SLP will will provide physical rehabilitative services to patients under specific medical orders as part of a coordinated team. Plan initial and subsequent treatment programs on the basis of clinical findings. Participate in clinical conferences to share and coordinate the plan of care for patients. Other duties as assigned. Legacy provides services solely to the senior population in Skilled Nursing Facilities, CCRCs, free standing Assisted Living and Independent Living Communities. We provide services under contract arrangement at skilled nursing facilities and in our numerous Medicare certified out-patient clinics. Therapeutic techniques and programs incorporate principles of rehabilitation and healthy aging, emphasizing home and community independence. Our services are both rehabilitative and preventative with a strong component of health and fitness education. 2. Current license in state of practice 3.
