Overview: The Agrace Social Worker position is a wonderful opportunity to help meet our mission of enhancing quality of life for those facing serious illness. This position coordinates direct and indirect patient-focused care with a dedicated and collaborative interdisciplinary team and maintains accountability for all social work practice, patient outcomes and referral to community resources.
This position will be traveling into facility partners in the Waukesha area to provide support.
Responsibilities: This position ensures a patient and family centered focus while providing social work assessments, interventions, evaluations, documentation and coordination of care.
The ideal candidate will be compassionate, independent, organized and enjoys a variety of patients and diagnoses. This position travels locally and works in both patient homes and health care facilities.

  • Bachelor’s degree in Social Work or related field, from an accredited program required; Master’s preferred.
  • Certification/Registration/Licensure: Current, unrestricted certification in Social Work.
  • 1 year social work experience in a health care setting required.
  • Previous experience with terminally ill, grief and/or bereavement counseling desirable.
  • Valid driver license and automobile insurance.
  • Must have access to automobile in good operating condition.
