The social worker should have the ability to complete initial and ongoing assessment of the patient and family’s response to the diagnosis, living situation and discharge planning needs, make timely referrals to appropriate community services based on identified needs that assure continuity of care through the discharge planning process. This may include but not limited to rehab placement, SNF, homecare, LTACH, DME and infusion care. The social worker will also provide support to patients and families as needed regarding adjustment to illness, changes in living situation, disability, hospitalization and death and dying.
Masters Degree from an accredited School of Social Work.
LSW or LCSW preferred.
