We offer medical detox as well as residential addictions treatment for active duty and retired military service members in our North Stonington location. Our clinic in Groton, CT, offers partial hospitalization and intensive outpatient treatment, and is conveniently located across the street from The Inn at Trails Corner, a new 180 bed dormitory-style sober community for adult men and women who attend our outpatient day treatment programs. The Inn offers overnight accommodations, full-meal service, indoor/outdoor recreation options, a workout room, and computer center for all clients. We accept private insurance, state-funded plans and self-pay arrangements.
The Medical Detox Social Worker Essential Responsibilities:
  • Develop and implement discharge plans, service referrals, case management and follow-up for an assigned client caseload.
  • Provide education components of program, under direction and supervision of a licensed Clinician, based on areas of specialty, client needs, and community history.
  • Coordinate service provider activities.
  • Provide psycho-educational groups focused on substance abuse and mental health education and/or education in an established area of expertise.
  • Identify problem areas, service gaps, and barriers to services for clients.
  • Document the client record in accordance with all regulatory and program requirements. Complete all documentation required by federal, state and accrediting agencies as well as military commands.
  • Act as a liaison to client, family, and aftercare agencies.

Qualifications

Social Worker Qualifications:
  • Master’s Degree from an Accredited School of Social Work required.
  • Experience working with adults in an inpatient and/or residential setting preferred.
