The Hospice & Palliative Medical Social Worker assists patients and families by providing resources for care to enhance the patient's quality of life during adjustment, planning, and after end of life care.

Position Qualifications:

  • 1 year of prior social work experience preferably in Hospice, Long Term Care, Home Care, or Acute Care environments
  • LSW required
  • Bachelor's Degree Required
  • Licensed Master's Level Social Worker Preferred
  • License in Ohio Required
  • Medicare guideline experience preferred


Job Responsibilities:

  • Assess and document patient/family social, emotional, financial, and psychological needs as well as resources on the Comprehensive Assessment
  • Assist patient and/or family with establishing DNR, advance directives, and/or DPOA or Healthcare proxy as requested
  • Assess and arrange patient placement such as Long Term Care, Assisted Living Facilities, and GIP as dictated by patient/family needs
  • Work in both Hospice and Palliative care Programs
  • Office is located in the Akron/Canton area and travel to the office is required.

Shift:

  • Monday through Friday 8:30 AM to 5:00 PM with a backup on-call rotation that includes evenings, weekends, and overnights.
