Annual Salary Range: $45,000 - $67,700

Hiring Salary Range: $48,000 - $51,000


SUMMARY:

  • Provide contracted CADI/DD case management services to clients of Catholic Charities.
  • Work respectfully with older adults, people with disabilities, persons in poverty, and people of diverse racial and cultural backgrounds.
  • Responsible for assisting clients with access to supportive services, helping clients set goals for themselves and their families, and providing ongoing care coordination to clients and their families.
  • Able to work effectively with clients with complex psycho-social concerns including mental health issues, unstable housing, unemployment, lack of medical insurance, dysfunctional relationships, and legal concerns.
  • Provide assessment services and short-term counseling, and skill building techniques to individuals.
  • Work collaboratively with outside agencies to provide behavior management support and consultation.
  • Participate in divisional, team and program staff meetings and other meetings as assigned.


MINIMUM QUALIFICATIONS:

  • Bachelors of Art or Science in Social Work or Nursing.
  • Professional Licensure approved by the State of Minnesota (e.g. LSW, RN).
  • Experience working with individuals with developmental disabilities.
  • Prefer experience in case management and/or knowledge of waiver services.
  • Experience with people of culturally diverse backgrounds.
  • CPR/First Aid Certification required, or in lieu of current certification, successful completion of CPR/First Aid training and eligibility for certification testing
