This position is a full-time, 40 hour per week position. Work hours are Monday through Friday, 8:00 AM to 5:00 PM. No weekends or holidays.

We are seeking a Social Worker to:

  • Join a dynamic, multi-disciplinary HIV clinical care program which is funded by the national Ryan White HIV/AIDS program.
  • Utilize both clinical and community based interventions to assist a diverse patient population facing complex medical and psychosocial needs.
  • Anticipate challenges and barriers that make it difficult for patients to stay engaged in HIV care and help improve self-management of their illness within a medical case management framework .
  • Support adult and pediatric patient populations.

Education :

Minimum – Master’s Degree in Social Work.

Work Experience :

Preferred – Two (2) years prior hospital or community-based social work experience.

Licenses and Certifications :

Minimum –

  • Certified Advanced Practice Social Worker (CAPSW) OR
  • Certified Independent Social Worker (CISW) OR
  • Licensed Clinical Social Worker (LCSW).

Physical Requirements :

Sedentary: Ability to lift up to 10 pounds maximum and occasionally lifting and/or carrying such articles as dockets, ledgers and small tools. Although a sedentary job is defined as one, which involves sitting, a certain amount of walking and standing is often necessary in carrying out job duties. Jobs are sedentary if walking and standing are required only occasionally and other sedentary criteria are met.
