We are currently seeking a Social Work Supervisor to join our Ambulatory Care Management team. This fulltime role will work days. Specifically seeking candidates with 2-5 years’ experience as team lead/supervisor and 3-5 years’ experience in the ambulatory clinic setting and/or community setting. This Ambulatory Social Work Supervisor will provide direct supervision and support for Social Workers in ambulatory primary care and specialty care clinics at Hennepin Healthcare. Primary job functions include: 1) Provide consultation and advising to the outpatient social workers in patient cases with ethical, legal, or clinical complexities, 2) Identify and educate the outpatient social work team on relevant legislative or state statute policies, procedures, or protocol updates that impact their work with HHS patients, 3) Manage department staffing and provide coverage as needed, to maintain minimum level of social work support to outpatient clinics.
