Responsibilities

Summary:

The incumbent collaborates with the multidisciplinary team to provide care for adult and pediatric septic patients. The individual provides clinical and professional expertise to focus on health promotion and disease prevention in a manner that is consistent with the Mission and Philosophy of the Sisters of Mercy.
Essential Functions:

  • Provides leadership for the sepsis program
  • Member of the Sepsis Alert team
  • Collaborates with hospital educators to develop and implement sepsis focused education.
  • Ensures evidence-based protocols and therapies are the standard of care followed within the facility
  • Collaborates with medical and clinical staff to provide multidisciplinary care to septic patients
  • Collaborates with Quality Management to communicate real time feedback to clinicians regarding missing bundle elements and required documentation
  • Collaborates with the nursing team to develop and implement plans of care including individualized teaching plans for patients and families
  • Analyzes data trends to assist with documentation, coding, clinical care, and improved outcomes.
  • Participates in rounding on patients with identified sepsis and proactively responds to patient needs to improve patient outcomes and positively impact overall patient experience

Qualifications

Minimum:
  • Current California RN license
  • Bachelor of Science in Nursing or other related field
  • Three years of recent critical care, and/or ED experience
  • Current BLS and ACLS from the American Heart Association

Preferred:
  • Masters degree in health related field
  • Analytical skills for collecting and analyzing data
  • Thorough understanding of the Core Measure/CMS Sepsis Bundle requirements, and remain current on any changes or updates to either initiative
  • Very comfortable communicating with physicians and other nursing staff
