POSITION: Turning Point Diversion Program services provide the full spectrum of community services necessary to attain the goals identified in each person’s treatment plan and Individual Services and Support Plan (ISSP). The program treatment team provides a collaborative relationship between the individual and when appropriate their family members or significant others. The program is comprised of a multidisciplinary team with diverse skills, expertise, and knowledge to help address each individual’s unique needs and challenges. Each person served is assigned a mental health clinician and case manager who act as a care team to support the individual. Other members of the treatment team include licensed vocational nurses, peer support specialist, education and employment specialist, substance use disorder specialist, criminal justice specialist, and a psychiatrist. The Turning Point Diversion Program is a “whatever it takes” and “meeting the individual where they are” program. The program is Assertive Community Treatment (ACT) and Full Service Partnership (FSP) model program supporting persons served in reaching their own vision of wellness and recovery. Services are delivered as an outpatient mental health program serving individuals diagnosed with a Serious Mental Illness (SMI), and at risk of being hospitalized, homeless, and/or incarcerated. The program focuses on individual strengths and abilities to successfully gain independence and self-sufficiency in the community. Individuals are assisted with community reentry and empowered to achieve a variety of goals. Services are provided to adults residing in Fresno County. Services can be delivered in the clinic, home, and community based-settings in collaboration with the person served. Services may also be delivered via secured virtual and telehealth applications and by telephone. Services are provided Monday through Friday between 8:00am and 5:00pm. Services are available for crisis and after hour emergencies 24 hours a day, 7 days a week.
